Step 1: ENROLL

Please speak with one of our specially trained diet coaches that will answer all of your questions and sign you up for the program. Our experts will provide you with all of the information you will need to determine which program is right for you. If you have previously spoken to one of our experts, you may sign up online and instantly receive your required documents.

Step 2: PRE-QUALIFICATION, AND REQUIRED DOCUMENTS

1. Wellness Blood Test - Our physician will order your required blood testing and send you to a laboratory draw center in your area. This information will be provided to you via email from Nu Image Diagnostics.

2. Completed Health History - A complete and accurate health history is vital to qualify for our program. You will need to fill out the form completely and fax it back to our office at 888-831-4818.

3. Sign all Required Forms - There are several important forms that will need your review and signature which include: HIPPA Privacy and Patient Bill of Rights. It is important to make sure you sign all of the documents so your pre-qualification won't be delayed.

Step 3: PATIENT REVIEW & APPROVAL

Once the pre-qualification process has been completed, your file will be reviewed by one of our medical directors. Once the physician has reviewed and approved your test results, your weight management plan can be executed and your medication prescribed. Your HCG will be shipped directly to you by an FDA Approved US Pharmacy and billed seperately in the amount of $150.00 + s&h.
